Is IELTS 4 a good score?

Not for most purposes. ielts.org describes band 4 as a Limited user whose basic competence is limited to familiar situations, with frequent problems in understanding and expression. It is below the 5.5 starting point of IELTS's own guidance table for courses, below Australia's Vocational English (5 in each skill), and a 4.0 in Listening falls short of IRCC's CLB 4 line of 4.5.

Moving up from band 4

The step to band 5 means moving from 'basic competence' in familiar situations to coping with overall meaning in most situations (ielts.org). Because the overall band is the average of four skills rounded to the nearest half band, lifting your weakest skill moves the overall fastest. See IELTS 4.5 for the next half band.

Sources for the thresholds: IRCC, Home Affairs Vocational English and Competent English.
