ECFMG's OET Medicine requirement
ECFMG requires every Pathways applicant to reach a satisfactory OET Medicine score, whatever their native language, language of medical school or citizenship, with no exceptions (ECFMG OET page). IELTS, TOEFL and PTE are not mentioned on ECFMG's certification or pathway pages.

One administration, retakes and timing
All four minimums must be met in one test administration. If you miss any sub-test, you retake all four and must pass them together; ECFMG sets no limit on the number of retakes, subject to scheduling.
For the 2027 Pathways, OET results count if taken on or after 1 January 2025. ECFMG advises 2027 Match applicants to test on or before the last scheduled December 2026 date. OET on paper or on computer at a venue is the default; OET@Home is only for applicants with no physical test venue in their country.

ECFMG Certification also requires an eligible medical school and passes in USMLE Step 1 and Step 2 CK (ECFMG Certification). The communication skills requirement is met by completing an ECFMG Pathway, which includes the OET Medicine score, or by a valid pass of the former Step 2 CS. OET is one part of a Pathway: the Pathway application itself must also be approved.

Does ECFMG accept IELTS?+
No. ECFMG's communication skills requirement is met through OET Medicine as part of a Pathway, and its certification pages do not mention IELTS.
What OET score does ECFMG require?+
At least 350 in listening, reading and speaking and 300 in writing, all in one test administration.
Is PTE accepted by ECFMG?+
PTE is not mentioned on ECFMG's certification or pathway pages; OET Medicine is the required test.
Can I combine OET sub-test scores from two sittings for ECFMG?+
No. If you miss any sub-test you must retake all four and pass them in a single administration.
