What the form needs
The waiver form covers the knowledge of language and Life in the UK requirement. Appendix KoLL 4.13ZA says it must be completed by your GP (or a GP at your practice) or a GMC-registered consultant who has met you in person and supports the request because your condition would prevent you meeting the requirements for the foreseeable future.
Gov.uk's exemptions page says to include original copies of current relevant medical reports with the form.
| Usually considered | Not normally accepted |
|---|---|
| Long-term illness or disability that severely restricts learning English | Temporary illness such as depression or stress |
| A mental condition that prevents learning English to the level | Illiteracy |
| Deafness, no speech, or a speech impediment limiting communication | Long residence in the UK |
How caseworkers decide
The caseworker guidance says each case is decided on its merits and caseworkers consider whether the condition prevents both parts (English and Life in the UK) or just one. A condition may make the Life in the UK test unreasonable while English can still be shown, and test centres can cater for some disabilities.
Form AN guidance says that if your illness responds to treatment, you are expected to prepare and meet the requirements; only a condition that prevents you permanently is considered.

ILR exemption does not carry over
If you were exempted on medical grounds at ILR, the caseworker guidance says you must make a separate exemption request for naturalisation. Gov.uk's exemptions page also says to submit an exemption form again for citizenship.
Frequently asked questions
Can any doctor fill in the form?+
For ILR under Appendix KoLL it must be your GP, a GP at your practice, or a GMC-registered consultant who has met you in person.
Is anxiety enough for an exemption?+
Form AN guidance says temporary illnesses such as depression or stress would not normally be grounds. A permanent condition is needed.
Can I get extra help in the test instead?+
Yes. Gov.uk says you can make special requests when booking a SELT, for example extra equipment or help accessing the location.
Does the exemption cover the Life in the UK test too?+
It can cover either or both parts. The caseworker considers each part separately.
