Ball State TOEFL minimums by admission type
From Ball State's English proficiency and conditional admission page, which applies to all applicants educated outside the US.

How Ball State handles TOEFL scores
Official scores must be sent electronically by the testing agency to school code 1051. English proficiency scores are good for 24 months. Ball State accepts TOEFL iBT MyBest scores as well as scores from a single test date, and accepts reports on both the new 1 to 6 and the previous 0 to 120 scales. Your programme or department may set different requirements.

If your TOEFL score is below Ball State's level
The Academic Bridge Program lets students slightly below the minimum begin degree classes while studying part time in the final level of the Intensive English Institute. Below that, conditionally admitted students enter the Intensive English Institute and must meet the English requirement before starting a degree programme. Since spring 2024 a language test score has been required for conditional admission.
Requirements vary by programme and change between admissions cycles. The score that binds you is the one on your Ball State programme page and in your offer letter, so check it there before you book a test.
