The three English stages for partners and parents
The family visa knowledge of English page says you must pass at least A1 for your first visa, and you can choose a higher level. If you passed A1, you need at least A2 in speaking and listening to extend after 2.5 years. If you passed A2, B1, B2, C1 or C2, you can reuse that result, as long as the certificate has not been withdrawn. If you were given an exemption first time, you need to pass A1.
A B1 or higher pass can be used again when you apply for settlement after five years, if the test is still on the approved list. Settlement on the family route needs "at least level B1 in speaking and listening" or a degree taught or researched in English (gov.uk ILR family).

Approved tests at A1, A2 and B1
Gov.uk says partner, parent and settlement applications need a test of speaking and listening (SELT guidance). The approved speaking-and-listening tests and pass marks, from the gov.uk lists for tests taken outside the UK and in the UK, are below. A four-skill test such as IELTS for UKVI (4.0 in each skill for B1) is also on the list.

No test is needed if you are applying as a child, as an adult coming to be cared for by a relative, if you have been in the UK on a family visa for five years and are extending as a partner or parent, if you are over 65, or if a physical or mental condition prevents you from meeting the requirement (gov.uk).
Nationals of Antigua and Barbuda, Australia, the Bahamas, Barbados, Belize, the British overseas territories, Canada, Dominica, Grenada, Guyana, Jamaica, Malta, New Zealand, St Kitts and Nevis, St Lucia, St Vincent and the Grenadines, Trinidad and Tobago and the USA are also exempt. A degree taught in English (UK-awarded, or confirmed by Ecctis) is accepted instead of a test.

Frequently asked questions
Which IELTS do I need for a UK spouse visa?+
IELTS Life Skills at A1 (speaking and listening) is the IELTS test built for this. Gov.uk also lists IELTS for UKVI, but that four-skill test is more than A1 requires.
Is IELTS Life Skills pass or fail?+
Yes. The gov.uk approved list gives the minimum grade for IELTS Life Skills A1, A2 and B1 as Pass.
Should I take B1 straight away?+
Gov.uk lets you take a higher level. A B1 or higher pass can be reused for the 2.5-year extension and for settlement, as long as the test is still on the approved list and the certificate has not been withdrawn.
Does the spouse visa English test expire?+
For a new application the SELT must be awarded in the two years before you apply. For reuse, gov.uk allows an A2 or higher result at extension and a B1 or higher result at settlement if the certificate has not been withdrawn.
